Poi teams are still working through during the Covid-19 lockdown. We have had to update our IT skills and while we can’t physically visit, we are available via email, phone or Zoom. We are still doing PPA’s, still here to answer your questions, and offer you support as you care for your palliative clients and whanau.
Just a reminder that ongoing regular use of the assessment tools (SPICT, AKPS and Phase) will continue to help you identify clients who will benefit from anticipatory palliative planning. Having a PPA in place will reduce unnecessary hospital admissions, and anxiety for family looking after loved ones at home and help your care planning. You will find the resources readily available on this website.
New Poi function: PMS Integration
The Poi PPA form is now able to integrate with Medtech, My Practice and Evolution systems. The installation link and guide can be found here
For Residential Care and GP practices not using one of these systems, a new web portal has been developed to send PPAs and receive PAS responses. If you have not yet received information with user name and password, please send us a message here to be set up.
The Palliative Outcomes Initiative (Poi)
Poi is an Auckland wide approach to primary palliative care.
Patients are identified in the community who are in the last 6-12 months of life and then strategies put in place to improve this time for them and their family.
The initiative centres around a Family / Whanau meeting, which then produces a plan for the lead clinician to implement, supported by the local hospice.
